CRU Cards for Winter Sports Athletes CRU CARDS: If your son or daughter is planning to try out for a winter sport, he/she must have a CRU Card prior to trying out each season. CRU Cards are given out by the athletic office and are a way for us to track all the necessary paperwork (that used to be done after your child has started the season) prior to the start of the season. Requirements are: Physical dated after April 15, 2017 ImPact Baseline Testing Complete (to be done every-other year. Winter sports required in all sports except for bowling.) Stay tuned for times at the end of October that athletes can sign up to take this test. Academic Eligibility & Transfer Eligibility – to be completed by the athletic office Final Forms We have partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students online. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information whenever possible, for each of your students, saving you time. You will review your information once per school year to verify information is current and sign the forms. Once you have registered, you can track the progress of the CRU card simply by logging in!
